Foodservice Establishment Questionnaire - Fats, Oils, & Grease

Thank you for taking the time to fill out our FOG questionnaire.  This is the first step in helping North Conway Water Precinct to maintain the health and reliability of our sewer collection and treatment systems.  Working together, we can ensure that our pipes stay clean, are less prone to clogging, and your business isn't interrupted by sewer backups.  After filling out the questionnaire below, you will be contacted by Precinct staff with instructions on the next steps.

What is the name of your food service establishment?
Enter the name of the person authorized to speak with North Conway Water Precinct with respect to your fat/oil/grease practices.
What is the title/role of the person authorized to speak with NCWP?
Enter the phone number of the Authorized Representative. NCWP will only use this phone number to communicate about the FOG program.
Please describe what you currently do to manage fats/oils/grease. Examples include kitchen policy/procedure, dish rinsing/washing procedures, and grease trap cleaning/maintenance procedures.
Select whether or not you have a grease trap or oil/grease separator.
Where is your grease trap located? Examples: Under the sink, Under the floor, outside underground, etc.
